What Are Special Needs Plans?
Special Needs Plans (SNPs) are a type of Medicare Advantage plan specifically designed to serve people with particular health conditions, dual eligibility for Medicare and Medicaid, or those living in certain institutional settings. Unlike standard Medicare Advantage plans, SNPs tailor their benefits, provider networks, and drug formularies to the unique needs of the populations they serve.
SNPs are available to Medicare beneficiaries who meet specific eligibility criteria. Depending on the type of SNP, you may qualify based on your income and benefit status, a qualifying chronic condition, or your living situation. Each plan is required to have a Model of Care approved by the Centers for Medicare & Medicaid Services (CMS) that outlines how it will address the specialized needs of its members.
Because SNPs focus on specific populations, they often provide more targeted benefits than general Medicare Advantage plans. This can include specialized care coordination, condition-specific drug coverage, additional benefits like transportation and over-the-counter allowances, and access to providers who have experience with your particular health needs.
Three Types of Special Needs Plans
Each type of SNP is built to serve a different population with distinct healthcare needs and eligibility requirements.
D-SNP (Dual Eligible)
Designed for individuals who qualify for both Medicare and Medicaid. D-SNPs coordinate benefits between both programs, often providing lower out-of-pocket costs, additional benefits like dental and vision, and access to a care coordinator who helps manage services across Medicare and Medicaid.
C-SNP (Chronic Condition)
Built for individuals who have one or more severe or disabling chronic conditions, such as diabetes, heart failure, chronic lung disease, or end-stage renal disease. C-SNPs provide specialized care management, tailored formularies, and provider networks focused on managing your specific condition.
I-SNP (Institutional)
Created for individuals who live in institutional settings such as nursing homes, skilled nursing facilities, or long-term care facilities. I-SNPs provide coordinated care within the facility, often with reduced cost-sharing and benefits designed around the unique needs of institutional residents.

Eligibility depends on the type of SNP. For a D-SNP, you must qualify for both Medicare and Medicaid. For a C-SNP, you must have a qualifying chronic condition such as diabetes, heart failure, or chronic lung disease. You can enroll in a SNP during the Annual Enrollment Period (October 15 through December 7), during your Initial Enrollment Period when you first become eligible for Medicare, or during a Special Enrollment Period if you qualify. Not necessarily. Many SNPs have low or zero-dollar monthly premiums, similar to standard Medicare Advantage plans. In fact, because SNPs are designed for specific populations, they often provide additional benefits at no extra cost. D-SNP members in particular may have reduced cost-sharing because of their dual eligibility for Medicare and Medicaid.
SNPs have their own provider networks, so it is important to verify that your current doctors and specialists participate in the plan's network before enrolling.
